Hospice of Pamlico County is located at 11146 NC Hwy. 55E, Grantsboro, NC,  between Tideland EMC and Cahoon’s Hardware and Sports Shop. 

Hospice of Pamlico provides care for the terminally ill Pamlico County citizens with life expectancy of six months or less. Hospice offers emotional support, symptom management and palliative comfort care that make it possible for the patient to remain at home with  family and loved ones in the last days.   

Referrals: By patient, family member or family friends, however, the attending physician must sign the treatment plan and declaration of terminal illness.  

Cost:  Hospice services' costs are covered by Medicare, personal insurance or private pay.  No patient is refused services due to inability to pay.  Hospice also is funded by memorials, donations, bequests and agency programs.

Services: Patient’s plan of care is provided by and through the Hospice interdisciplinary team and ordered by the patient’s attending physician.   The hospice team is composed of hospice RNs, the attending physician, a volunteer physician, chaplain, social worker, home health aides, nutritionist, Physical Therapist, patient/family volunteers and bereavement counselor.     

            Nurses - Assistance and support 24 hours-a-day/seven days a week

            Chaplain - Spiritual support as requested by patient

            Home Health Aides - Personal care needs of the patient

            Patient volunteers - Respite care of patient and family as needed.

            Nutritionist - Diet management and tools provided by a nutritionist

            Physical Therapist - Services provided as needed.

            Social Worker - Emotional and social support given by LCSW

           The Bereavement Counselor - Bereavement services are available for the year following the death of the patient.  Bereavement newsletters are offered to each family for the 1st, 3rd, 6th, 9th months and anniversary of the patient’s death.  Newsletters are also sent on Easter and Christmas holidays.
