CRAVEN COUNTY HOME HEALTH The Craven County Home Health is part of the Craven County Health Department and provides professional health care services in the home. Provided in the home with a Doctor’s order: 1. Registered Nurses a. Visits are made to do evaluations, treatments such as wound care, IVs, home teaching, or monitoring of medications and symptoms, as ordered by the Dr. 2. Speech Therapy, Occupational Therapy, and Physical Therapy 3. Medical Social Workers a. Visits are made to do evaluations and referrals to other community services. 4. Nutritionists 5. Home Health Aides
a. They
assist with bathing and personal care and change bed linens. Coverage for these services are provided by: 1. Medicare (If homebound) 2. Medicaid 3. Third Party Insurance 4. Private Pay Referrals can be made through the physician or discharge planner at the hospital. Upon referral, the nurse will make a visit to do an evaluation of what services are necessary to care for the patient in the home. She will also call the insurance company to determine the coverage held by the client. 6/2008 |
